This application is also referred to as Form I-102.  The Application for Replacement Nonimmigrant Arrival-Departure Document is to be used by non-immigrants whose original arrival/departure document has been lost, stolen, or destroyed. It is also used to replace a Form I-94, Nonimmigrant Arrival-Departure Record, Form I-94W, Nonimmigrant Visa Waiver Arrival- Departure Record, or Form I-95, Crewman Landing Permit. Replacement may be due to lost, stolen, or destroyed documents, or due to incorrect information on the documents.  If the incorrect information is the fault of USCIS, there is no fee for the replacement document.

 Form I-102 is also used for non-immigrants applying for a new non-immigrant arrival–departure document.

Common I-102 Answers and Tips

This form should NOT be used to replace an I-94 issued by U.S. Customs and Border Protection.  If you need a new Form I-94 which was issued at a port-of-entry, you need to contact the nearest Customs and Border Protection office to ask about their procedures.

You must have a physical address, rather than a post office box, when you complete Form I-102.

If any information in your application, or supporting documents, is in a language other than English, you must have the document or information translated into English and the translator must certify he is competent in both languages.
