Form I-102 is used to apply for replacement or initial Non-Immigrant Arrival Departure Document. You can use Form I-102 to request a new or replacement of an incorrect, lost, stolen, or damaged Nonimmigrant Arrival- Departure Document.
Application
Form I-102 can be downloaded from the USCIS website. You must complete all items on the form. If you need more space, use additional sheets of paper. If you are using additional sheets, make sure you indicate the item number and sign each page. A fee of $320 must be submitted by way of cashier’s check or money order payable to Department of Homeland Security. The completed form along with necessary documents and fees must be sent to the USCIS office having jurisdiction over the state you are located in.
Information
You must provide the certain personal information including your full legal name, address in the US, alien registration number (if any) and social security number (if any). You must also provide the following information:
- Your current non-immigrant status
- Date on which you last entered the US
- Date on which your authorized stay in the US ends
- The number at the top of your arrival departure document if you have been issued an arrival departure document when you arrived in the US.
The form check the appropriate reason for your application – your arrival departure document is lost or mutilated or has incorrect information or was not issued at the time of your arrival in the US. The form must be signed by you. If the applicant is under 14 years of age, the form can be signed by the applicant’s parent or guardian.
Evidence
Your application must be supported by evidence of your entry into the US. If you are applying because your arrival departure document is mutilated, you must submit the original document with your application. If your document is lost or misplaced, you should attach a copy of your document. If you do not have a copy, you must submit a copy of the biographic page from your passport and a copy of the page indicating your admission to the US as a non-immigrant or other evidence of your admission. If you do not have any such evidence, you must provide a full explanation stating why you cannot give any of the above evidence, along with a copy of evidence of your identity and copies of any evidence in your possession to substantiate your claim and a copy of the police report if your document was stolen.
