As primary evidence, applicants may submit one of the following along with Form DS-11:
(1) a citizenship certificate;
(2) a certificate of naturalization;
(3) a certification of birth or a consular report of birth abroad;
(4) a certified birth certificate issued by the state, county, or city; or
(5) a previously-issued U.S. passport in good condition.
Applicants without their U.S. birth certificate or lacking primary proof of U.S. citizenship may present secondary evidence such as public records. Examples include a combination of the following: a physician's record of post-natal care, an early school record, a census record, a hospital birth certificate, and a baptismal certificate.
