A form visa is a document or documents that one must fill out in order to obtain visa standing within the desired country. When an individual hopes to gain employment, study abroad, or attend a particular university or given destination within a country, they must fill out a form visa. There are a wide variety of forms that one may be required to fill out when it comes to seeking and ultimately gaining a visa, it all depends on the type of visa. The length of time that one hopes to seek the visa for can also have a bearing on the forms one must fill out. Obviously before one gains visa to a country there are some rather extensive questions that must be answered about the person's background and intent for seeking out the visa, plus the intention of the stay as well.
